NORWALK, Conn. — Theres an old tradition in standard medical circles of dismissing so-called alternative medicine as everything from witchcraft to nonsense. The move, the first by a major U.S. health-care company, suggests that the industry is giving some credence to consumer claims that certain kinds of alternative medicine work. In fact, many insurers already cover some types of alternative medicine, such as therapy by chiropractors — although few cover less conventional treatments like herbology, in which herbs are used to treat diseases. “Its really driven by consumer demand,” said Hassan Rifaat, manager of Oxfords alternative-medicine division. Analysts and industry executives say the market for alternative medicine amounts to billions, though the exact amount is up in the air. Norwalk, Conn.-based Oxford pegs the market at about $50 billion; others estimate a more conservative $18 billion to $20 billion. Today, most of that money is paid out of pocket by consumers. Yet, the size of the alternative-medicine market doesnt necessarily translate into a boost for the bottom line of health- care companies or insurers. For example, employers that subscribe to Oxford plans can sign up to provide coverage for alternative treatments by paying a 3 percent increase in premiums, the company said. Even if all Oxford customers added that coverage, it would represent only a small portion of the $1.73 billion in premiums that the company collected in 1995. “Its not an area with any real commercial significance,” said Paine Webber Inc. analyst David Lothson. That could change — or thats what alternative-care providers hope. HMOs now cover 60 million Americans, while another 80 million are covered through other forms of managed care and traditional medical insurance. “As more services are covered, the more alternative treatments people will try,” said Neal Heller, chairman and chief executive of Natural Health Trends Corp., which runs schools that train massage therapists and operates clinics that offer alternative-health treatments. Plenty of people are already convinced that alternative treatments work. The American Chiropractic Association reports that about 20 million people visit chiropractors each year, though only half of the nations HMOs cover those services. Oxford said its recent decision was based on member demand after a tremendous response to an article on the subject in its member magazine. The company, which provides coverage to about 1.4 million people in the eastern United States through its HMOs and other plans, isnt just limiting coverage to treatments now widely seen as effective, such as visits to chiropractors. Included in its new network of 1,000 alternative-medicine specialists are acupuncturists, naturopathic doctors, massage therapists, yoga instructors, clinical nutritionists and registered dietitians, along with a division to sell herbs and holistic devices. It said it also plans to add other services, like the Chinese martial art taichi. Some other health care companies, such as American Western Life Insurance Co. and Kaiser Permanente Health Plans, offer a limited range of alternative-medical services, but none have developed a full network and benefits plan comparable to Oxfords. “Mainstream HMOs and insurers are skeptical,” said Natural Healths Heller. Indeed, more Americans have taken a greater interest in their own health and preventive care, particularly as HMOs strive to cut costs by placing limits on certain types of treatments. “They feel as though modern medicine has failed them,” Heller said. While coverage of alternative treatments is on the rise, it will take several years before theres wide acceptance among benefit plans and insurers, analysts and executives said. When that happens, Heller said, his clinics will be ready.

This film is visually entrancing (the sequences set at the track are sufficiently stimulating, chaotic and noisy with furious angle cuts to engage your basic attention-deficient kid) and often slyly comic (because there are only cars in this world, tractors do the work of farm animals and one popular prank is tipping over tractors) and generously garnished with characters that can be personable (the wonderfully naive beat-up pick up voiced by Larry the Cable Guy is as endearing and appealing as any Disney character since Thumper). But it lectures, often heavy handedly, about a lost America, dated here to the mid-1950s, a world of caring neighbors, a slow down, small-town world of simple pleasures like a slow drive in the country, where love was expressed in batted eyes and stammering speech. Granted that 1950s is a confection of Hollywood and perhaps it is an exercise in pointless nostalgia to mourn something that never really existed, it works here. When Lightning McQueen, the showboat hot-shot race car (Owen Wilson), loses a big race (his daredevil antics on the track cost him the victory) and decides brashly to eschew his racing team to head alone to the season-ending showdown race for the Piston Cup, he gets lost on a stretch of back highway, the mythic Route 66, in the deserts somewhere on the way to California. He ends up in a lonesome town, Radiator Springs, itself a cross of intersections bypassed long ago by the interstate and now settling into dust. When Lightning ends up impounded in the town and then sentenced by an unforgiving old-timer judge (Paul Newman) to community service–he must repave the stretch of broken-down highway his race car has damaged–the young kid comes to understand, slowly, the simple values that his media-driven celebrity lifestyle and virtually impenetrable narcissism have never shown him: friendship, trust, honesty. Happily, the judge turns out to be himself a relic of past racing glory and with his help and his patient (although wonderfully gruff) instruction the kid learns a thing or two about handling cars. When Lightning returns to race, he does so richly educated in the importance of tradition, the value of modesty, and the sterling quality of friendship. I would not dream of spoiling the end of the film, but in the end we learn, as with all truly significant sports films, what happens in competition pales next to the significance of what happens off field. With Lightnings newfound sensitivity, the town finds its way to a renaissance that is as believable as it is heartwarming.

CHRISTMAS shoppers will welcome the news that Barclaycard has teamed up this week with 17 retailers to offer discounts of 10 per cent or more to cardholders who use their Barclaycard for Christmas shopping. The retailers hope to persuade shoppers to spend money before Christmas rather than waiting for the sales. The offer should also help boost Barclaycard, whose share of the credit card market has been dropping steadily. This is not the first time credit cards have offered cashbacks and discounts instead of points and air-miles. Goldfish, the credit card run by British Gas and HFC Bank, offers users discounts on gas and electricity bills from British Gas Trading, or the chance to set savings against BT bills and the TV licence. It offers points for each pound spent, which can be swapped for vouchers at Asda, MS and Boots. Alliance Leicester broke new ground last March with their Moneyback credit card, offering a rebate of 0.5 per cent on spending up to pounds 3,000 a year, rising to 1 per cent on anything over that. In January, 400,000 cardholders will get average rebates of pounds 25. On top of that, eight retailers, including BP, MFI and Currys, give cardholders instant discounts equal to the rebates from the Alliance. The incentive seems to work: cardholders use their card 12 times a month, four times the industry average. The Birmingham Midshires discount card launched this year offers tiered cashbacks ranging from 0.33 per cent up to pounds 3,000 a year, to 0.75 per cent and then 1 per cent on spending over pounds 7,000. In May the Amex Blue card launched, offering 1 per cent cashback off the value of all purchases made with the card, which will rise to 5 per cent next month only. The annual pounds 12 fee takes a little icing off the cake. Barclaycards campaign ends on Christmas Eve. If it wins extra business for the retailers and Barclaycard it may lead to something more permanent. The other cashback cards may also decide to improve and extend their offers. However, experts say its too soon to rule out a negative health impact of cell phones. A 13-nation study on the relationship between cell phones and brain tumors is under way. Looking at freight transport, there was a significant increase in maritime and railway transport, while the volume of road transport remained roughly at the level from last years second quarter. After the volume of maritime transport fell by a fifth in the first quarter, it rose by 14.5% in the second quarter of 2007, year on year. The volume of transport has become more volatile in recent years, but the volume of goods carried has not contracted significantly relative to the global conjuncture witnessed in the industry in 2005. Maritime transport is one of the minor transport modes in Slovenia. Railway freight transport grew by 14.9% in the first quarter and continued to grow strongly in the second quarter. Harbour transport recorded a modest increase, while road freight transport did not increase in year-on-year terms in the second quarter, partly because of the very high volume of road transport seen in the second quarter of 2006. Data also show that the expansion in this mode of transport, boosted by the abolition of restrictions upon Slovenias accession to the EU, has moderated somewhat this year Slovenia is near the top among EU countries according to the volume of road freight transport per capita. The assessment refers to the volume of freight transport expressed in tonne kilometres, undertaken by freight vehicles registered in a given country. It measures carriage by domestic freight vehicles in the country of registration and abroad, and not the volume of transport within the territory of a given country. In 2006 by far the most per capita road shipments were performed by carriers registered in Luxembourg (18,766 tkm; see graph). Slovenian carriers (with three times lower freight transport per capita) were ranked second and performed 58.9% more transport than the average carrier in the EU. The high intensity of road freight transport in Slovenia is the result of the surging growth rates of road transport, particularly in 2004 and 2005 (27.9% and 22.5%). From the second quarter of 2004 to the second quarter of 2007, the volume of shipments carried by freight vehicles registered in Slovenia increased by 53.8%. Domestic transport rose by 23.9%, while international transport climbed by as much as 63.8%. Within international transport, the largest (3.2-fold) increase was recorded in cross-trade transport and cabotage (transport performed in another country). The well-developed shipping industry in Slovenia is attributable to Slovenias favourable position at the crossing of European corridors V and X. Transport on these corridors is increasing, and Slovenian carriers are exploiting new business opportunities. With Slovenias entry into the EU, a number of administrative barriers for carriage within the EU were removed for Slovenian carriers (especially the procurement of a limited number of licences). Slovenia also negotiated the possibility to perform cabotage in the EU-15 countries, which the Eastern European countries that joined the EU together with Slovenia will only be allowed to do after the end of a five-year period.
TRANSPORTATION: The state Conference of Mayors (NYCOM) has endorsed the $2.9 billion transportation bond act that will be on the Nov. 8 ballot. The Albany group said the projects it would fund would be good for the fiscal and economic health of communities NYCOM believes the bond act funding will expand the capacity and safety of NYs transportation system, while also promoting energy-efficient forms of transportation, said Peter Baynes, executive director of the mayors group. The Business Council of NYS has also endorsed it, despite voicing reservations about adding to NYs already high debt load. Half the funding will go to transit projects in the Metropolitan Transportation Authority region and the other half to bridges, roads, airports and other transportation facilities outside NYC.

Pregnancy Morning Sickness Caused By Thyroid Changes OSAKA, JAPAN: A team of Japanese researchers has found that changes in thyroid function occur during the first trimester of pregnancy that correlate with the severity of early-morning nausea and vomiting known as “morning sickness,” but that the changes are no cause for alarm and do not indicate thyroid disease. All women were free of clinically detectable forms of thyroid disease. The pregnant women were categorized based on the severity of morning sickness: no symptoms, nausea only, and nausea and vomiting. The researchers found that not only were blood concentrations of T4 significantly greater, and TSH levels significantly lower, in pregnant than in nonpregnant women, higher blood levels of T4 (and hCG) “were especially marked in subjects with nausea and vomiting.”
My best friend Joan had three agonizing pregnancies. The term “morning sickness” doesnt describe what she went through. The nausea and vomiting — not just in the morning but all day and for months — made her question each decision to have another child. Everyone knows of at least one woman like Joan who got nowhere near the recommended calories or nutrients during the first part of her pregnancy — the time, were told, when nutrition is most critical — but whose babies didnt seem to suffer any consequences. Why, then, all the nervousness about a vegetarian pregnancy? Even when vegetarians are eating far better than the Joans of the world, why are they routinely told that they should eat meat, fish, eggs or at the very least, dairy products — or theyll never get the full spectrum of nutrients they need for an uncomplicated pregnancy and a healthy baby? The answer is that there is no simple answer. Because nutrition is not an exact science, the uncertainty leaves room for personal bias, disagreement and even fear. The Recommended Dietary Allowance (RDAs) say that you need a daunting 1,200 milligrams (mg.) of calcium every day; the World Health Organization (WHO) says you need only 500 mg. Why would international guidelines be less than half of the American guidelines? After all, were always given the impression that those Third World mothers are malnourished; dont they need more nutrients? Which guidelines does your doctor follow? Who can you believe? And how on earth can anyone consume the RDAs of 1,200 mg. of calcium, 30 mg. of iron and upwards of 80 grams (g.) of protein a day?

C ONTAINMENT If I was the author in 1947 of a doctrine of containment, it was a doctrine that lost much of its rationale with the death of Stalin and with the development of the Soviet-Chinese conflict. I emphatically deny the paternity of any efforts to invoke that doctrine today in situations to which it has, and can have, no proper relevance.While agreeing on the desirability of resisting communist expansion, Kennan and others disagreed on whether the doctrine remained relevant, and how and where to implement it. Their disputes have often rested on fundamental differences about the capacity of American power, about the extent of American interests beyond western Europe, and especially about the nature of the communist threat. The last issue has raised many questions. Was the threat subversion, revolution, military aggression, economic encirclement, or some combination? With the exception of Yugoslavia, was world communism controlled by Joseph Stalin even after the successful Chinese revolution in 1949? After the death of Joseph Stalin in 1953 and after the obvious Sino-Soviet split in the early 1960s, did the nature of the communist threat sometimes change, even well before the collapse of the Soviet Union in 1989#x2013;1991? At times from the early 1960s, according to the proponents of containment, was the threat primarily China and wars of national liberation in the Third World, and not the Soviet Union mostly in the developed world? Was American policy in the 1990s and continuing into the twenty-first century in dealing with Cuba, North Korea, and the Peoples Republic of China often the policy of containment?Beyond these important issues, scholars, as well as politicians and policymakers, have raised other questions: whether the doctrine in 1947 was new or necessary, whether it was ultimately self-defeating, whether it was active or passive, and whether it did or should have endured as American policy into the late 1980s and early 1990s and perhaps later. Many of the questions about containment, if it is interpreted as the general course of American foreign policy, become the basic questions about that policy itself, from Harry S. Trumans administration to the first years of President George H. W. Bushs and possibly beyond.

Amistar President Stuart Baker said the company decided to get out of the contract manufacturing business and to develop its distributed delivery networks business that services the retail pharmacy industry. SMS and Amistar plan to make presentations to Amistars six largest customers in hopes of retaining them at SMS Sorrento Mesa plant. Privately held SMS has some 25 regular, large-volume customers, and many smaller customers that use the plant to produce new products.

Exercise and Cancers: Research Opportunities for Health-Related Fitness ScientistsRepresentatives from the American Cancer Society (ACS), the National Cancer Institute (NCI), and the Centers for Disease Control (CDC) will challenge AAHPERD members to contribute to cancer prevention and control, describing the most promising research opportunities and resources to support investigation of the relationships between cancers and exercise. The presentation will be based on a synoptic article being developed by the ACS and the Office for Health Promotion of Northern Illinois University. Ruth Corcoran of ACS will describe a proposed collaborative research agenda, suggesting that two sets of consensually developed guidelines are needed to promote such research: one set to facilitate dissemination of what experts already know to the public and providers (including exercise specialists); and a second set to facilitate further collaborative research. Speakers from CDC and NCI will outline what is known about these topics, and how exercise scientists can contribute to further development (see accompanying abstracts). Presenters will recommend a process to develop research priorities, involving cancer experts, public health scientists, and relevant interest groups in AAHPERD. The magical wedding date of the year — 07/07/07 — has passed us by, but to those who figure in figures when setting the date for a special occasion, all is not lost.
When the previous statement on foreign exchange was discussed at the Accounting Practices Committee we were of the view that the statement should include a definition of the date of the transaction due to the confusion surrounding this concept. We consulted with our legal colleagues and were informed that, if an order was placed for goods or services, the date the order was accepted was the date of the transaction. We attempted to get this entrenched in the statement but failed at the APB level. After much confusion, it was generally agreed that the cost of the inventory in the above example would be R65O if no cover contract was entered into or if cover was taken but hedge accounting was not permitted. It was also generally agreed that if cover was taken and hedge accounting was permitted, the cost of the inventory would be R6oo (the spot rate at the date the deal was made) plus RS (being the cost of the premium attributable to the inventory, i.e. Ri5-Rio) or R6o5. Then along came AC 133 with its embedded derivative in a host supply contract concept. It stated that if the deal was done in a currency that was not the currency of one of the parties to the transaction and the product subject to the transaction was not routinely bought and sold in the currency in which the transaction was entered into, there was an embedded derivative in the contract that had to be separated from the host contract. In the above situation, the embedded derivative would have been a liability of R45 (R6,6o - R6,is). The only way that could understand this concept was to see the embedded derivative liability as an expression of sadness that the deal was entered into in dollars and not in Rand. If the Rand went the other way, it would have been an asset, i.e. an expression of happiness. (A banker once asked me jokingly if he could lend money to a company based on the value of its embedded derivatives in its supply contracts!)

ABSTRACT This treatise concerning moisture precipitation under freezing conditions proceeds from similarity between the coil-frost formations of a recent coil-frost testing program and coil-frost formations frequently found in freezers coincident with frost accumulations around doorways or elsewhere within the room. Calculated data involving dehumidification alternatives applicable to the two (or more) temperature/humidity environments typical of freezer/anteroom complexes are presented, which reveal the paramount role of applied psychrometrics with respect to air-side engineering at freezer temperatures and which, in conjunction with modern control-system technology, prepare the way for a future treatise that promises significant logistics-related benefits through the integration of all refrigeration loads of these high-usage facilities into an overall refrigeration-system design within which coil-frosting need not occur. ASHRAE Technical Committee 10.8, Refrigeration Load Calculations, was formed from a Task Group circa 1983. Coincident with its formation, the ASHRAE director of technology directed questions to the new TC of contradictory refrigeration-system performance where snowlike coil frost was seen regardless of the location at which the ice-crystals first appeared. All questions involved freezer/anteroom complexes. Contradictory instances occurred where the addition of artificial sensible-heat load–with no other change–was accompanied by significantly lowered freezer temperature. Other contradictory instances of lowered freezer temperature occurred simply in response to raised suction temperature, sometimes dramatically, typically a week or so following the change. Such observations in conjunction with one member (Cole 1989) having recently published a paper describing aerosol emissions commonly seen during coildefrosting, which had expanded upon earlier coil-icing research (Stoecker et al. 1983), promrted an early decision by the new TC to prepare and ask the society to fund a testing program for the measurement of freezer heat gain due to coil defrosting under the various circumstances of coil design and operation customarily found in industry. Following forums, seminars, and symposia considering such a program, a Work Statement was eventually approved in 1997 that became ASHRAE RP-1094, A Study to Determine Heat Loads Due to Coil-Defrosting–Phase II.

By contrast, the anti-legalization camp argues that violent crime would not necessarily decline in a legalized drug market. In fact, there are three reasons why it might actually increase. First, removing the criminal sanctions against the possession and distribution of illegal drugs would make them more available and attractive and, hence, would create large numbers of new users. Second, an increase in use would lead to a greater number of dysfunctional addicts who could not support themselves, their habits, or their lifestyles through legitimate means. Hence crime would be their only alternative. Third, more users would mean more of the violence associated with the ingestion of drugs. These divergent points of view tend to persist because the relationships between drugs and crime are quite complex and because the possible outcomes of a legalized drug market are based primarily on speculation. However, it is possible, from a careful review of the existing empirical literature on drugs and violence, to make some educated inferences.

A review of the marketing literature in both the traditional (offline) and new era (online) environments was undertaken to compare and contrast marketings role in both types of organisation. Insights derived from this review formed the basis for a series of in-depth interviews with leading marketing professionals in Ireland.
